2. List of medications

3. Contact lenses or information about your contact lenses (empty boxes or packages are acceptable)

4. Solution currently used (including eye drops)

5. IntakeQ forms completed to the best of your ability

6. Your medical insurance card

7. ​Vision insurance card (if available)
